Before you fill out the online Chemical Form the Rouge Education Project team would like to make you aware of a few changes to the form.  In an effort to gain a better understanding of the data being collected by schools we have added two new features.  The first addition is a confidence scale located in the monitoring portion of the form.  For each of the nine tests you are now required to decide how confident you are of your results.  Please use the confidence scale below to determine your confidence level.  If you did not perform a test please select Did Not Perform Test from the drop down menu. The other change we've implemented is to track the test kits used to perform each test.  If the test kit you used is not provided as an option please select Other and record the test kit you used in the comment section at the end of the form.  If you did not perform a test please select Did Not Perform Test from the drop down menu.
